| Health & Malaria Inspector Grade-II | B.Sc. with Chemistry, 01 Year Diploma in Health / Sanitary Inspector. |
| Lab Assistant Grade-II | Diploma in Medical Laboratory Technology (DMLT). |
| Dialysis Technician | B.Sc. and Diploma in Hemodialysis. |
| ECG Technician | Degree / Diploma in ECG Laboratory Technology / Cardiology / Cardiology Technician / Cardiology Techniques. |

For General / OBC / EWS: ₹ 500/-
For SC / ST / EBC: ₹ 250/-
For All Category female: ₹ 250/-
Refundable Fee: General / OBC: ₹ 400/- and Other candidates: ₹ 250/- will be refunded after appearing in Stage I Exam.
